A construction crane collapsed Friday on New York's Upper East Side, smashing into a 23-story apartment building as it fell to the ground, killing two construction workers and seriously injuring another who was a pedestrian. The two men who died were the crane’s operator Ramadan Kurtas, 27, and Donald Leo, 30. One body was brought out of the rubble, placed on a gurney and covered in a white sheet. A construction worker knelt over the stretcher, gently stroking the sheet. City building department records show that inspectors stopped crane work twice last month. One of which was for unsafe operation and lack of proper permits. Investigation reveals that the top part of the crane snapped off and fell against the building around 8 a.m. Witnesses described a loud boom, then a cloud of smoke and debris. A police spokesman said the injured workers were in serious condition at New York-Presbyterian Hospital/Weill Cornell Medical Center. One was being treated for a chest wound, the other for cardiac arrest. The general contractor on the project is Leon D. DeMatteis Construction Corp. of Elmont on Long Island. Spokesman Don Miller said Friday that Sorbara Construction, a subcontractor, was in charge of operating the crane. A woman who answered the telephone at Sorbara Friday said no one was available to comment. Cranes at the site had generated several complaints in the neighborhood, including reports that safety barriers were breached and heavy loads passed over the heads of pedestrians, according to city building department records. Inspectors found that most of the concerns were unwarranted, but they did temporarily order one crane at the site to stop all work on April 23. Building Department records also said officials halted work after a crane on the site failed a “load test” on April 22. The crane passed a second test, however, the next day, and no violation was issued. On March 15 about 2 miles to the south of Friday’s incident, contractors building a 46-story condominium near the United Nations were trying to lengthen a crane when a steel support broke, killing seven people. A four-story town house was demolished and several other buildings were damaged. A buildings inspector had found the crane to be safe a day before the collapse; another inspector resigned after his arrest on charges of lying earlier about inspecting the same crane. In April, the city's buildings commissioner resigned, under fire over a rising number of deadly construction accidents that have left more than 26 construction workers dead in the past year. Since then, the city has added extra inspections at building sites and required that its staff be on hand whenever the towering cranes were raised higher, a process known as a jump. Efrain Vincente-Cruz, 30, of Chandler was sentenced to 14 years in prison Wednesday for a collision that killed a 59 year old woman. Vincete-Cruz had a blood alcohol content of .243, three times the legal limit when he crashed head on into a vehicle being driven by Marjorie Weeks at Rittenhouse and Ellsworth Roads in Queen Creek. A witness reported seeing Vincete-Cruz driving erratically prior to the fatal collision on December 17.

Brenda Sullivan was sentenced to 20 years in prison for aggravated child abouse for keeping her 17-year-old adopted son caged in her home. Sullivan pleaded guilty in January to three counts of aggravated child abuse. Prosecutors agreed to drop lesser child neglect charges. The teen weighed 49 pounds when child welfare workers found him in 2005 in what appeared to be a cage. Sullivan told a judge at the time that Ohio authorities told her to keep the boy, who had severe medical and emotional problems, in a crib. "There's only one conclusion when you look at the medical evidence in this case, and that is that she literally starved him," prosecutor Julie Schlax said. Two other children, 13-year-old twins the Sullivans adopted as infants, both testified they were kept in similar cages. Sullivan's husband was also arrested, but died in January 2007 while awaiting trial. Sullivan's lawyer, said he didn't think prison was the right option because she does not pose a threat to society. Chandler police broke up what they say appears to be a one person fraud, forgery and counterfeit enterprise after a Casa Grande woman tried to cash a forged check at a supermarket. According to Chandler police, Shari Denise Ballard, 50, had five computers and templates for United States currency, counterfeit money, forged checks, check writing programs and evidence in the manufacturing of fake Arizona driver licenses in her home. Ballard was arrested and booked on 13 counts of fraud and forgery. Sgt. Rick Griner, Chandler police spokesman, said there were test documents and some of the final products looked nearly authentic and could be passed if a clerk wasn’t paying attention. Investigators will try to determine as many victims as possible. So far, investigators believe Ballard worked alone because all the forged identification and checks were in her name. Ballard tried to pass at Food City, 1005 N. Arizona Ave., but it was less than $200. An employee at the store called police and patrol officers made a traffic stop and found evidence with Ballard that allowed detectives to draft a search warrant for her home. An Arizona Department of Corrections officer and five family members have been arrested for trafficking methamphetamine from Mexico for distribution in the U.S. Noe Ruiz, 31, who worked at Lewis Prison in Buckeye for seven years, was arrested after a seven-month investigation that yielded 20 pounds of methamphetamine Tuesday night and another 35 pounds in other states including South Dakota and Ohio, said Maricopa County Sheriff Joe Arpaio. Deputies estimated the 20 pounds of methamphetamine to have a street value of $2 million. Investigators also recovered weapons, 12 pounds of cocaine and $400,000 in cash in Arizona and other states. Deputies arrested Ruiz's father, Domingo Ruiz, 55, after pulling him over for failing to use a turn signal near 83rd Avenue and Indian School Road. They found four pounds of methamphetamine in a hidden compartment in the car and a weapon. Deputies then searched five residences, including the younger Ruiz's, where they found money-counting machines, Arpaio said. Arpaio said Domingo Ruiz was the leader of the operation and his five children were distributing the drug. "It sound like the old mafia days," he said. "Except this crime family happens to be a real family." Also arrested were the younger Ruiz's siblings: Maria Ruiz-Ochoa, 27; Humberto Ruiz, 29; Ruben Ruiz, 22; and Javier Serrano-Garcia, 19 . Police arrested Two El Mirage teenagers Wednesday in connection with a homemade explosive device that went off in their school cafeteria during lunchtime. The two teens, ages 16 and 17, who are juniors at Dysart High School in El Mirage. In less than a month, this is the third explosive-device incident to emerge from the Dysart Unified School District. The other two happened at Willow Canyon High and Valley Vista High, both in Surprise and involving the detonation of "soda bottle" bombs. The teens were detained on two felony charges of interfering with an educational institution and misconduct involving weapons. They are being held at the Maricopa County Durango Juvenile Detention Center Police said the two students brought a "small homemade incendiary device" to school and ignited it during lunchtime. The explosion caused a disturbance, and school officials extinguished the device. No one was hurt in the explosion. The school's resource officer impounded the device and called the El Mirage police. The device was then sent to the state Department of Public Safety crime lab. "The El Mirage Police Department and Dysart High School take these incidents very seriously," Louis said. "Our department will always take a zero-tolerance approach to anyone who introduces any harmful or volatile substances onto our school campuses or interferes with the safe operation of our school campuses." The investigation is ongoing and other charges may follow. On May 1st two Valley Vista freshmen were detained after a plastic bottle filled with a mixture of household cleaners exploded in the school’s outside courtyard. A week before, a Willow Canyon High student set off a similar chemical bomb as a prank, which ended in the evacuation of part of the school. Arizona Legislators are trying to once again toughen Arizona laws against drunken driving and boating, but this time without softening a penalty for some offenders. On Thursday the Senate assembled a drunken-driving bill that doesn't include a provision that prompted Gov. Janet Napolitano to veto a previous measure. That measure would have cut the time that first-time DUI offenders would have been required to use ignition interlocks. The current requirement mandates placement of the device a year, but the bill would have shortened it to six months. Ignition interlocks require drivers to blow into a tube to measure their blood-alcohol level before they can start their cars. The new proposal still would require that first-time offenders convicted of operating a boat while intoxicated serve 10 consecutive days in jail. It also expands circumstances in which a drunken driver's license is suspended for 90 days after a fatal accident. A Tucson judge has tossed out alcohol breath tests in 49 DUI cases. City Court Judge Thomas Berning ruling says the breath tests are inadmissible because the company that makes the machine hasn't made its inner workings available to the defense. A Tucson defense attorney says the ruling could have widespread implications. One of the defense attorneys involved in the cases, says there are 50 to 70 pending cases before other judges that were waiting for Berning's ruling. A teenager was booked on charges of criminal damage- tagging for scrawling 18 Mesa neighborhood vehicles with black ink on Saturday. Mesa Police report an officer caught the teen and two others by surprise about 12:15 a.m. when he drove up on them in the 1000 block of South Grenoble with his headlights turned off. The officer got one of the teens immediately as two ran away. A second teenager was stopped a while later. Minnesota state lawmakers have reached a settlement for the Victims of the Interstate 35 bridge collapse last year. The collapse killed 13 people and injured many others. State Sen. Ron Latz and state Rep. Ryan Winkler reported that the deal totals $38 million and will be presented to the Legislature for approval. The deal would expedite compensation to victims who chose to take their portion of the settlement and waive their right to sue the state of Minnesota. Victims will be able to collect up to $400,000.00 and victims whose damages exceed $400,000.00 will be able to collect from a supplemental pool of over $12 million dollars. There are over 182 claimants, 13 were killed and 12 catastrophic injury cases. The bridge collapsed on the afternoon of August 1, 2008 during rush hour. Dozens of cars tumbled into the Mississippi River. The National Transportation Safety Board is investigating the bridge and found that 16 gusset plates, which strengthen the junctions of steel beams, were fractured. The gussets were found at eight joints in the main center span which made a domino effect. The final report should be ready by the end of the year, he said.

Prosecutors have charged former NBA star Dennis Rodman, 46, with domestic violence for allegedly hitting his girlfriend at a Los Angeles hotel on April 30. Rodman was charged Wednesday with spousal battery, dissuading a witness and brandishing a deadly weapon, all misdemeanors according to Los Angeles City Attorney Nick Velasquez. Rodman was arrested at a hotel April 30 after Gina Peterson called hotel security. Police say she suffered injuries to her arm. Darren Prince, a spokesman for Rodman said he had too much to drink when he got into an altercation with Peterson. Prince says the couple is still dating. "We look forward to a successful resolution of this misdemeanor matter," said Rodman's attorney, Paul Meyer. Attempts to find a phone listing for Peterson were not successful. Richard Mulligan, 45, the Director of Economic Development for Chandler was arrested for the assault of his ex-girlfriend. Mulligan was arrested Sunday after an altercation with an ex-girlfriend outside his Chandler home. Police reported that Mulligan allegedly jerked Sonya Banna out of her vehicle and threw her on the ground. Mulligan denied the allegations but a man walking his dog nearby saw the incident and backed Banna's version of what happened, police said. Chandler San Marcos Justice Court records show that last month Banna, 45, sought an injuction against Mulligan claiming he assaulted her in 2006 and has been harassing her with e-mails, threats, phone calls, and stalking since 2004. According to the restraining order, Mulligan pulled Banna into his car and punched her in the thigh in July 2006. Police were called but, "I backed out of doing anything else because I still loved and cared about him and thought it was an isolated case," Banna said in the petition. Banna claims that their relationship had been troubling since 2004 and that she was threatened by Mulligan if she filed a restraining order she would lose her job because of his position with the City of Chandler. Mulligan filed for a restraining order the day following his arrest claiming the former girlfriend's unannounced visits to his home and "loud screaming or throwing up" in his front yard disturbs his two teen-age sons and neighbors. He said in his petition that Banna collaborated with his ex-wife to get the earlier injunction. "It sounds like I was harassing him and wouldn't leave his house," Banna said. "I'm in the car, in the street. If you want me to leave, why yank me out of the car and why be standing in the street?" She also said she was not taunting him with her cell phone, but that he yanked her out of the car to get her phone to see who was calling her. Mulligan has said he won't comment on the case and has referred questions to his attorney. Mulligan is set to appear in city court on the criminal charge June 18. He is also scheduled to appear in San Marcos Justice Courton Thursday for a hearing on Banna's injunction. Mulligan, 50, signed a "separation and release agreement" with the city and plans to submit a letter of resignation, walking away from his $140,891 a year job. Stephan Doimas, 61 of Chandler was arrested Tuesday after allegedly using fraudulent schemes to obtain people’s tax returns. Chandler police say Doimas targeted people who were in need of financial help targeting mostly single females. Doimas fraudulently arranged for the tax returns to be wired to his own bank account and would then transfer the money to other accounts. Doimas is not a CPA but does have a tax preparer number and has been doing taxes for nine years. Financial Crimes Unit arrested Doimas on suspicion of fraudulent schemes, theft, multiple counts of forgery, and threats. Chandler Police estimated that damages exceeded $12,000 but expect figures to rise as the investigation continues. Doimas indicated during questioning that he completed tax returns for approximately 40 people and had their refunds wired into his bank account. Investigators confirmed the tax returns he did file for numerous people contained fraudulent information. Police continue to try to locate potential victims. Detectives said they will be continuing to investigate to determine the total value of loss and number of victims as well as the seizure of some bank accounts, police said. Michael Bame, 23, of Avondale, was taken into custody Saturday for allegedly discharging a firearm within city limits. Bame was attending a wedding reception at the Pinnacle Peak Patio restaurant in Scottsdale when the incident occurred. Bame became angry with his wife when she refused to get into the vehicle claiming he had too much to drink. Bame showed his anger by firing a gun into the air.Bame was also arrested on suspicion of disorderly conduct and endangerment about 9:30 p.m. No one was injured from the gunfire, and Scottsdale police said they did not see any damage in the nearby neighborhood. A 17-year-old Tucson teen was arrested during Catholic mass for allegedly robbing a nearby Macy’s store just minutes before. Tucson police entered St. Joseph’s Catholic Church and arrested the teen. The teen had pulled a handgun on security officers at Park Place Mall. Tucson police would not release the identity of the minor, who was booked into the Pima County Juvenile Court Center. Macy security officers spotted the teen and a young woman take two jackets. The teen was confronted and that is when he displayed a pistol. The teen then ran from the mall pointing the pistol at a security guard who attempted to stop him. An off-duty officer working at the mall spotted the teen and followed him after hearing a police call over the radio. Savannah Sutherland, 23, the daughter of the founders of Scottsdale based Cold Stone Creamery was sentenced to 3 ½ years in prison May 8, 2008 for the fatal collision that killed Genovena Tepec-Juarz, 36, in East Mesa on March 20, 2006. A Maricopa County Superior Court judge also punished Sutherland with five years of probation and 1,000 hours of community service. She had pleaded guilty to manslaughter, criminal damage and driving while intoxicated. On March 20, 2006, Sutherland rear-ended a scooter driven by Genovena Tepec-Juarez, 36, in east Mesa near the intersection of Baseline Roadand Farnsworth Drive. In a turn of events that Judge Andrew Klein called very remarkable, the victim's family intervened so Sutherland would receive a shorter prison term than what was outlined in a plea agreement reached two months ago. Before being sentenced, Sutherland read a statement in which she took full responsibility for killing Tepec-Juarez. "I promise that for the rest of my life, I will try to make up for the tragedy I caused," she said. "And I hope to have the wisdom and strength to persuade others to avoid the mistakes I have made." Those mistakes were drinking four beers at a bar and driving away, as Sutherland later confessed to police. Her blood-alcohol content was recorded at 0.194, more than twice the legal limit. When Sutherland's 2003 Chevy Cavalier struck the scooter, the impact threw Tepec into the windshield. Up until the sentencing, Sutherland was under house arrest and the 24-hour supervision of her parents, Donald and Susan Sutherland, founders of the Scottsdale-based chain of ice cream stores. A 40 year old man was fatally injured Tuesday morning after being crushed by a 7,500 lb stack of Hay in Gilbert.
The unidentified man was air lifted to Scottsdale Healthcare Osborn with severe injuries after being pinned in the torso between the stack of hay and the bed of a flatbed truck by a hay squeezer. Two men were operating the hay squeezer which is used to transport hay onto a flatbed truck for transport. The operator of the hay squeezer did not realize the worker had been pinned. After discovering the worker had been pinned, the operator immediately removed the injured worker.
Gilbert Fire Capt. Rob Duggan said that criminal charges are unlikely and the incident was a "terrible accident".

After more than two years the former mental patient that had been stalking Uma Thurman, Jack Jordan has been convicted stalking and one count of aggravated harassment in a New York Court.
Jordan was accused of stalking and harassing Thurman by sending cards, love letters and showing up on her doorstep and movie sets.
Jack Jordan, a 37-year-old out-of-work lifeguard and pool cleaner, faces up to a year in jail.
Prosecutors say that Jordan fist became obsessed with Thurman in 2005. Jordan, who lives with his parents in Gaithersburg, Md., said he was involuntarily committed to a mental facility in late 2005 after being questioned about his obsession with Thurman. Jordan also sent cards and letters to Thurman while institutionalized.
He was eventually released, and started repeatedly showing up at her apartment last year. At the time of his arrest, he was living out of his car and working part-time as a lifeguard and pool cleaner.
The week long trial featured testimony from the Academy award-nominated actress, who testified in front of the jury that she was "completely freaked out" by Jordan's behavior. "It was almost like a nightmare; it was scary."
